In addition to the registering on this site, you will need to download, print and fill out the following forms from the Manual High School Web Site.  The easiest way to accomplish this is to Right-Click on each of the links below and choose "Save Target As..." from the context menu.  Then save each of the forms on your local hard drive.  From there you can open each one, print it out and then fill it out.
